1. Confirm the date of purchase, the location and the product.
2. Complete the on-line rebate/offer application form. You will be asked to enter full mailing address and email address so you can check your rebate status on-line.
3. Print out a summary of your application form. Mail the summary form, your cash register receipt with the item circled and any other proof as required by the offer to the address provided.
