Purchase any one case of cans (12 or 24) of Prescription Diet^® Dog Food and receive $25. Purchases must be made between September 15, 2024 and December 31, 2024. All requests for refunds must be received on or before January 30, 2025.

1. Complete the online rebate form (name, mailing address, etc)
2. Attach an image of your original dated cash register receipt from where you purchased the product.
3. Attach an image of the UPC barcode from your case of Hill’s^® Prescription Diet^®.
