Confirm the date of purchase, the location, and the product.
Complete the on-line offer application form and submit. A mandatory requirement is to enter both your full mailing address and e-mail address.
Print out a summary of your application form. Mail the summary form, your original cash register receipt with the item circled and the UPC code written on the receipt (if it is not printed), as required by the offer to the address provided.
